Nursing Division of the health department is located in the lower level of the
Courthouse. Our office entrance is located down the ramp at the northwest corner
of the building. We are open from 8:00 am until 4:00 pm, Monday through Friday.

IMMUNIZATIONS
INDIANA STATE
REQUIREMENTS
Indiana Law (IC
20-34-4) and Administrative Code (410 IAC1-1) require school children be
immunized against certain diseases or file a religious or medical
waiver. Below is a complete list of the minimum immunization
requirements for school entry for the 2011-2012 school years issued by
the Indiana State Department of Health.
SCHOOL VACCINE
REQUIREMENTS FOR 2011-2012 SCHOOL YEAR
Immunization
requirements for kindergartners and first graders include two chicken
pox vaccines (or written date of the disease signed by the child’s
health care provider). Immunization requirements for sixth through
twelfth grade students include a Tdap (tetanus, diphtheria and whooping
cough) vaccine, two chicken pox vaccines (or written date of the disease
signed by the parent), and a meningitis vaccine.
IMPORTANT:
Children under 18 years of age must be accompanied by parent of legal
guardian or another adult over 18 years of age WITH a letter signed by
the parent or legal guardian, naming the adult and child and giving the
adult permission to bring in the child.
A complete shot
record showing all immunizations received since birth is required. (Shot
records may be obtained by calling the school the child attends or their
healthcare provider.)
Children should
wear short sleeves and eat before coming to the clinic.

Two dose alternative adolescent schedule (Recombivax HB given at age

given on or after the child’s fourth birthday.
